Shared Care Model

We value longitudinal relationships between patients and their providers. Postoperative medical management, chronic disease optimization, and long-term preventative care are a collaborative effort. Our hope is to supplement a multidisciplinary approach to improve our patients' outcomes.

If you prefer to maintain postoperative follow-up after a defined global period, we are aligned with that. If extended orthopaedic follow-up is needed, we will communicate clearly. 

Ideal Referrals

  • Acute shoulder or knee injuries
  • Suspected rotator cuff tear, instability, or labral pathology
  • Painful or failed shoulder arthroplasty
  • Sports injuries in active adults
  • Patients seeking definitive answers after prolonged non-resolution or failure of conservative management. 

Pre-Referral Tips (Helpful but Not Required)

  • Recent plain radiographs when applicable
  • MRI reports or images if already obtained
  • Prior operative reports for revision cases
  • Relevant medical comorbidities and anticoagulation status

If these are not available, we will coordinate appropriately.
